Veno-venous Extracorporeal Membrane Oxygenation (V-V ECMO) allows to provide CO2 removal, oxygenation and lung rest in the most severe ARDS patients, thus minimizing ventilation-induced lung injury (VILI). In the last decades, a major technological and clinical development allowed a significant outcome improvement in patients treated with this technique and led to a consequent increase of its utilization worldwide. The chapter will discuss the physiologic rationale, the indications, and the clinical evidence of V-V ECMO and less invasive extracorporeal carbon dioxide removal systems.
