The paper reports the case of a 13-year-old girl who suffered from complicated appendicitis, which was treated by laparoscopic appendectomy. Subsequently, she developed a bilateral pleural effusion that required a chest drain. Pleural exudative effusion is a rare complication of perforated appendicitis caused by irritation of the diaphragm and by the passage of liquids from the abdominal cavity to the thoracic one due to pneumo-peritoneum and Trendelenburg position.
