This article examines the primary care reform in Italy, outlined in the National recovery and resilience plan and implemented through Decree 77/2022, by comparing the national framework with the Lombardy model. Drawing on a comparative analysis of national and regional regulations, the study explores the conceptual and organizational aspects of the reform, focusing on three dimensions – knowledge, integration, and participation – deemed crucial to understanding its most innovative features. While the national model emphasizes community-centred care, social participation, and the integration of social and health services, Lombardy’s approach prioritizes digital infrastructure, clinical efficiency, and hierarchical governance, often depicting citizens as passive recipients rather than active actors. The analysis highlights strengths and weaknesses of both approaches and reflects on the conditions needed to turn the reform into a genuine paradigm shift.
