We present MedIcona, a prototypical EMR that we developed to address some challenging requirements raised from influential GPs and recently discussed in different research fields that recognize the role of images, not only medical imaging, in medical practice and the value of allowing their freehand and free-text annotation. We make the point that innovative EMRs should encompass better communication oriented features, and allows for higher levels of end-user tailoring, content sharing and image annotation
