In this paper, we discuss the concept of InterPersonal Heath Record and its role in the most recent models of health service delivery: such a health record should integrate health-related information with communication and collaboration capabilities that could allow both patients and caregivers co-produce information and knowledge toward prevention and illness treatment and management. We then present MedIcona, a fullfledged demonstrator of the IPHR notion developed as free, open-source installation profile of Drupal 7. Its functionalities are aimed at enabling higher levels of user tailoring, content sharing and annotation among patients and their caregivers.
