The use of the BPTB graft is still considered the gold standard in the ACL arthroscopic reconstruction. The skin incisions used for the patellar tendon harvesting are described as: the standard longitudinal incision, the Levy & Byck variation and the double horizontal incision described by Mishra. From the year 2002 to the year 2003, we have treated over 60 patients for ACL reconstruction with BPTB graft, using a new skin incision technique for the surgical approach to the BTBP harvesting. This technique uses a multiple Z incision (with a 90° angle between each consecutive segment), that goes from the rotula apex to the tibial tuberosity. This incision provides a better view of the intraoperative field, a lesser post-operative pain over the harvested zone, a better range of movement in particular during the knee flexion, a better scar healing.
