Hip revision surgery varies from straightforward replacement of the polyethylene insert of an acetabular component to more extensive pelvic reconstructions. The restoration of correct joint biomechanics by means of the implantation of a stable prosthesis is the primary aim of the surgeon. The loss of bone mass can make reimplantation extremely complicated. Various solutions are currently available in terms of revision instruments and implants to address the technical difficulties of this class of operations. To obtain satisfactory results even in the most difficult cases, it is useful practice to be prepared in the theatre to apply a whole range of solutions, being clear as to what to do in every possible circumstance. Certain technical possibilities and modern approaches to hip revision as discussed in the literature are then analysed, particular attention being paid to some solutions that have been introduced relatively recently for replacing a prosthesis of the acetabulum.
