The vascular complications in a consecutive series of 400 renal transplantations performed from 1972 through December 1987 are described. No vascular complication was demonstrated in the 23 patients receiving graft from living related donors. In 29 (7,2%) patients the graft failed to function depending on vascular problems. Stenosis of the renal graft artery was angiographically evident in 9 patients (2,2%). Arterial and venous thrombosis occurred respectively in 18 (4,5%) and 2 (0,5%) cases and in all graftectomy was required. Technical and anatomical factors, the type of anastomosis together with atherosclerosis of the recipient and donor vessels play an extremely important role in the incidence of vascular complications. The availability of an arterial patch anastomosed end to side to the external iliac artery and the 'en bloc' harvesting technique seems to reduce the incidence of vascular lesions and make the anastomosis easier and more reliable in cases with multiple artery kidneys. The use of Ciclosporine A, as an immunosuppressor, not-withstanding some contrasting reported findings, did not prove to be a predisposing factor to vascular thrombosis

Abstract
