AIM OF THE WORK. The purpose of this paper is to describe the intentional replantation technique of lower molars when orthograde or retrograde treatment is not possible. MATERIALS AND METHODS. 8 patients (5 men and 3 women aged between 35 and 55 years) presented periapical lesions on second mandibular molars that could not be treated by orthograde or retrograde endodontic techniques. All teeth were extracted with atraumatic technique, then retreated with retrograde filling using IRM cement and finally replanted 15-20 minutes after extraction. RESULTS. Replanted teeth showed good primary stability and a complete resolution of periapical lesion was radio-graphically seen in all treated cases, Both spontaneous and provoked pain were absent. CONCLUSION. Even considering the risk of root fracture, intentional replantation showed until now a good reliability coupled with an absence of post-surgical symptoms in treated cases. Our experience, supported by literature data, encourage us to continue in the use of this approach when traditional techniques have failed or the risk of nerve damage is too high.
