I luoghi della nascita: la realtà delle case maternità in Lombardia. Place of Birth: evaluating Birth Centers in Lombardy

In the last several decades, there have been considerable changes in childbearing patterns worldwide. Out-of-hospital birth is increasing and Birth Centers represent the major alternative to obstetric unit. In Italy only the 0,1% of deliveries take place out of obstetric units, but Birth Centers are increasing now. We have few information about these birthplaces. The aim of this study was to describe Birth Centers in Lombardy. We conducted semi-structured interviews to midwives in charge. Six major themes were evaluated: terminology, structure and position, history and philosophy, Midwifery-led Model, management and accessibility. Interviews were audiotaped and transcribed. Results showed that Italian Birth Center are small private practices, managed by midwives with complete autonomy. Midwives adopt Caseload Midwifery Model and care is not limited to labour, birth and postpartum, but it includes midwifery, gynaecological and neonatal care.
