This thesis is focused on patient self-care and caregiver contribution to self-care as key elements in type 2 diabetes mellitus management. The uncovered association between patient self-care and glycemic control suggested to deeper investigate the phenomenon of diabetes management, not only from the patient point of view but also considering the role of the main informal caregiver. Accounting for the interdependence between behaviors performed by patients and caregivers, a dyadic approach was used to jointly consider patient self-care and caregiver contribution to it. Furthermore, to comprehensively describe the dyadic engagement in type 2 diabetes care, three distinct and unobserved dyadic patterns were identified according to dyadic average (i.e., how patient and caregiver do together) and incongruence (i.e., the magnitude and direction of the gap between the two members of the dyad) of each dyad. To get these results, multilevel models and a latent class analysis were performed, respectively. Moreover, the characteristics of each pattern and their association with glycemic control were investigated. Lastly, a methodological exploration of an alternative way to deal with dyadic data to be used to identify patterns of dyadic engagement in illness care was conducted. Interestingly, using simpler observed measures (i.e., mean and difference between dyad members) instead of coefficients predicted by multilevel models as input variables for the latent class analysis yielded the same patterns as the other approach. Overall, this thesis may provide useful suggestions to healthcare professionals and offer a starting point for further dyadic research in type 2 diabetes.

Questa tesi è focalizzata sul self-care del paziente e sul contributo del caregiver al self-care come elementi chiave nella gestione del diabete mellito di tipo 2. L’associazione tra self-care del paziente e controllo glicemico ha suggerito di indagare più a fondo il fenomeno della gestione del diabete, non solo dal punto di vista del paziente ma anche considerando il ruolo del suo principale caregiver informale. Tenendo conto dell’interdipendenza tra i comportamenti messi in atto dai pazienti e dai caregiver, è stato utilizzato un approccio diadico per considerare congiuntamente il self-care del paziente e il contributo del caregiver. Inoltre, per descrivere in modo esaustivo il coinvolgimento diadico nella cura del diabete di tipo 2, sono stati identificati tre distinti pattern diadici sulla base della media diadica (vale a dire, come si comportano insieme paziente e caregiver) e dell’incongruenza (vale a dire, l’entità e la direzione del divario tra il due membri della diade) di ciascuna diade. Per ottenere questi risultati sono stati utilizzati rispettivamente modelli a effetti misti e una latent class analysis. Inoltre, sono state studiate le caratteristiche di ciascun pattern e l’associazione con il controllo glicemico. Infine, è stata condotta un'esplorazione metodologica su un approccio alternativo ai dati diadici da utilizzare per identificare pattern di coinvolgimento diadico nella cura della malattia. È stato interessante notare che l’utilizzo di misure osservate più semplici (vale a dire, media e differenza tra i membri della diade) al posto dei coefficienti previsti daimodelli a effetti misti come variabili di input per la latent class analysis ha prodotto gli stessi modelli dell’approccio precedente. Nel complesso, questa tesi può fornire suggerimenti utili agli operatori sanitari e offrire un punto di partenza per ulteriori ricerche diadiche sul diabete di tipo 2.
