international studies (Haidet, et al, 2002; Hojat, et al., 2004) describe the process of loss of empathy in medicine student - less attention to patients' emotional needs or attention to the relational aspects of caring - and a decrease in emotional intelligence (Goleman, 2005). Quite often the ward experience is seen as the main cause of this relevant relational loss (Neumann, et al., 2011). In our research we assessed if and how the ward experience really has an influence on the relational attitudes in a sample of italian medicine students. The Jefferson Scale Physician Empathy (Hojat et al., 2001) and the Patient Pratictioner Orientation Scale (Krupat, 2000) were used as assessing tools with an open question on motivation to medicine in two studies: the first compared the profile of second (no ward experience) vrs fifth years student (4 terms of ward experience); in the second a longitudinal assessment was done with a subsample of 40 students. according to our results and contrary to the anglosaxon literature ward experience seem to have few if none effect on empathy and on patient-oriented attitude. two hypothesis explaining the results: a good tutorship in the university hospital and an early clinical experience in the second years (one week with the GP)

Studi internazionali (Haidet, et al, 2002; Hojat, et al., 2004) rilevano che con l’avanzare nel percorso universitario, negli studenti di Medicina diminuisce sia l’importanza attribuita agli aspetti relazionali nella cura del paziente sia il livello di empatia: elementi costitutivi, insieme alla motivazione, dell’intelligenza emotiva (Goleman, 2005), impalcatura di qualsiasi relazione. Spesso questo cambiamento viene associato all’esperienza di tirocinio nei reparti ospedalieri (Neumann, et al., 2011). Nella ricerca che presentiamo si è voluto verificare se l’esperienza di tirocinio abbia un’influenza sulle modalità con cui gli studenti si approcciano alla cura dei pazienti anche nella Scuola di Medicina di un Ateneo Milanese. Il protocollo di ricerca è costituito da due questionari self report, il Jefferson Scale Physician Empathy (Hojat et al., 2001) e il Patient Pratictioner Orientation Scale (Krupat, 2000) e da una domanda aperta sul perché hanno scelto di fare medicina. La ricerca è suddivisa in due studi. Il primo studio è svolto su 326 studenti frequentanti il Corso di Laurea in Medicina e Chirurgia dell’Università degli Studi di Milano Bicocca: 190 studenti al secondo anno (senza esperienza di tirocinio in ospedale), 136 studenti al quinto anno (con almeno 3 tirocini in reparti ospedalieri). Il secondo studio prevede un disegno longitudinale: 40 soggetti hanno compilato il protocollo al secondo anno e successivamente al quarto anno. Per il primo studio è stata svolta un’analisi multipla lineare, per il secondo è stata applicata l’analisi del T-test. L’esperienza di tirocinio non sembra influire sulle dimensioni prese in esame in questo studio. Questo fenomeno potrebbe essere messo in relazione con un atteggiamento funzionale dei tutori a cui questi studenti si riferiscono durante il tirocinio e anche con il fatto che hanno la possibilità di riflettere su questi aspetti già durante il secondo anno di corso.
