Several methods can be used to establish fetal lung maturity by testing for components of fetal lung secretions in amniotic fluid. Fetal pulmonary maturity should be viewed as a probability that is a function of gestational age, amniotic fluid analysis and clinical maternal/fetal conditions. Most methods have similar diagnostic indices, and the predictive value of a mature test ranges from 95% to 100%. On the contrary the risk of RDS for an immature test varies from 5% to 100%, depending on gestational age and result of the test.17 The choice and the sequence of tests should be based upon availability, presence or absence of contaminants, and physician and laboratory personnel preference. © 2010 Blackwell Publishing Ltd.
