Tolosa-Hunt syndrome is a rare type of painful ophtalmoplegia, almost exclusively monolateral, characterized by a series of clinical criteria; diagnosis is usually reach by an exclusion procedure. New contribution from MRI have recently allowed diagnosis to be based not only on clinical but also on neuroimaging hallmarks. In this article we review the history and the actual knowledge on Tolosa-Hunt syndrome starting from desciption of an unusual case report characterized by a bilateral involvement of the oculomotor nerves.
